Click here to print off the form.

All required portions of the form must be completed. This form can update your address or sign you up for mail ballots.

3. Returning the Form

Voter registration forms can be mailed or delivered to your local election office. Click here for your county election office address.

At this point in time, voter registration forms cannot be returned electronically nor is there an online portal to submit one. The law requires an original form to be submitted to your local election administrator.

For the November 6, 2018 Federal General, voters must register to vote by October 9 to vote at their polling place. For voters who  missed this deadline, please click on the Late Registration button below for  more information.
